Renew an Organization

If you would like to renew  an organization be sure to complete the Club Packet. To complete this document the club executive must initial all lines by each statement and obtain the proper signatures. In addition to the Club Memorandum of Understanding for Club Renewal, an up-to-date organization Constitution, Inventory of Club/Organization Authorized Originations, and roster of organization members needs to be on record in the Dean of Students Office.

Upon approval, dues need to be submitted to the Dean of Students Office. To collect dues please check out a money bag and receipt book from the Dean of Students Office. A receipt must be written and given to each individual club member. 

If there are any constitutional changes, member or executive officer status changes (by adding new members or members leaving the group) documentation needs to be submitted in a timely matter to the Dean of Students office with respective dues for new members.

Start an Organization on Campus

If you would like to start an organization at The California Maritime Academy, carefully review and complete the Club Packet and Memorandum of Understanding. Club Packets should be turned in to the ASCMA Offices to process and recommend for approval.

Forms

Below are the forms required to coordinate events, order supplies using organization funds, and fundraise for your organization. Please keep in mind that forms are due a minimum of 10 business days prior to an event start date. Some events may take longer to coordinate and you will be advised if your proposed event fall into this category. All forms require multiple signatures to process for approval; this formality ensures that your request is completed within ASCMA and University guidelines .  By submitting your request you acknowledge your agreement to the standards set forth by the Associated Student of the California Maritime Academy (ASCMA) and Cal Maritime.  We look forward to working with you!

ASCMA Event Request – This form is to be used for all student organization sponsored events on & off campus.  If vendor payments are required, the ASCMA Financial Request form will also be used.  If you are unsure whether or not your request falls into this category please contact the ASCMA Executive VP of Finance at as.vp.fin@csum.edu.

 ASCMA Event Budget Worksheet – This spreadsheet is to accompany all Event Requests.  This spreadsheet is fully editable so please tailor the categories to fit your needs.  If you have questions about this form please contact the ASCMA Executive VP of Finance at as.vp.fin@csum.edu

ASCMA Financial Request – This form is to be used for all item, vendor, and reimbursement requests.  If you are unsure whether or not your request falls into this category please contact the ASCMA Executive VP of Finance at as.vp.fin@csum.edu.

Liability Waiver - This document needs to be completed by individuals participating in any off-campus event and some on-campus events at the time of the event. You will be informed if you need to have it completed for on-campus activities. This document is not able to roll-over or be a blanket waiver for multiple events. Even if it is the same event, at the same location, at a different time during the year you will need to submit a new waiver. All waivers need to be submitted to the ASCMA Office on the first business day following the event.

Vehicle Rental - Complete this document if you would like to rent a vehicle for an organization's event. You are required to have a certified Defensive Driver through the CSU. The person completing the Defensive Driver Course must be 21-years-of-age and have a valid California Drivers License. Only certified drivers over 25-years-of-age may rent and drive 12 passenger vans. 

Attendance Sheet - Each individual participating in an event on or off campus needs to sign in on this sheet. The sheet needs to be submitted to the ASCMA Office the first business day following the event. 

Fundraising – If you would like to have a fundraising event on or off-campus please complete and submit this form to the ASCMA Office within 10 business days prior to your event request date. It must be processed through the Office of the Dean of Students and recommended for approval through the Office of Advancement.

Important Links

Defensive Driving for Students - In order to complete this course you need to be at least 21-years-of-age and have a valid California Drivers License. Upon completion please submit your Certificate of Completion and a copy of your California Drivers License to the ASCMA office before renting a vehicle.

Defensive Driving for Advisors – All advisors that would like to drive a van rented through the University must complete the CSU Defensive Driver Course. Upon completion please submit your Certificate of Completion and a copy of your California Drivers License to Stephanie Alvarez in the Dean of Students office.

Enterprise Roadside Information - Please refer to this document in case there is need of roadside assistance.

Executive Order 1074 – System wide policy prohibiting discrimination, harassment and retaliation against students and procedure for handling discrimination, harassment and retaliation complaints by students
